Targeted Case Management
Targeted Case Management is a Medicaid funded service that aims to eliminate barriers to treatment by addressing the social, emotional, educational, and vocational needs of clients. Targeted Case Managers work closely with the client and their family to assit in navigating mental health care, while advocating for their needs across domains of functioning.
Case Managers work with the client to provide care plans that include individualized goals and objectives aimed to support the entire treatment team and wrap services around the client to be successful in the mental health treatment journey.
Our team of licensed professionals offer quality counseling and mental health services, including individual, family and marital therapy, substance abuse treatment, grief counseling and more.
Experienced therapists use evidence-based practices to work closely with clients and families to help them identify, assess, and meet treatment goals. We are a Trama Informed Case agency with an emphasis on safety compassion, and understanding of the unique experiences of individuals and families.
Our psychotherapy office is child-friendly with a dedicated space for play therapy. We also have a team of school-based therapists partnering with local school staff to support the growth and success of our child and adolescent clientele.
In an effort to make our services accessible to all, we continue to offer appointments via telehealth.
